The purpose of the Chronic Disease and Injury Prevention Division of the Santa Clara County Public Health Department is to create and maintain healthy, safe and livable communities.
Chronic Diseases-such as heart disease, stroke, cancer and diabetes- are among the most prevalent, costly and preventable of all health problems. Leading a healthy lifestyle (avoiding tobacco use, being physically active, and eating well) greatly reduces a person’s risk for developing chronic disease.
Nationally, motor vehicle–related injuries are the leading cause of death for people ages 1–34, and nearly 5 million people sustain injuries that require an emergency department visit. The economic impact is also notable: motor vehicle crashes cost around $230 billion in 2000.
The Santa Clara County Public Health Department works with community partners to make organizational, system and policy changes to promote healthy, safe, livable communities.

May 24, 2010 Policy Forum
The Chronic Disease and Injury Prevention Division sponsored a Policy Forum attended by local policy makers, community leaders, advocates, and prevention and wellness experts, to discuss city and county ordinances and state legislation to support the prevention of chronic disease. The forum also served as the launch of the U.S. Department of Health and Human Services Communities Putting Prevention to Work (CPPW) tobacco initiative funded by the American Recovery and Reinvestment Act (ARRA) of 2009.
