When you are completing birth certificates, it is very important to use/reference the following:

• Birth and Death Registration Handbook (updated 6/07). If you do not have a copy or are missing pages, please re-order a new copy using this form.
• Medical Data Supplemental Worksheet (updated 01/06) should be used to complete the Confidential Portion of the birth and fetal death certificate.
• The Automated Vital Statistics System is the application used for preparing birth certificates. If you need to modify access for your employees, use this form.
