The Vital Records and Registration Office:

• Registers Santa Clara County births and deaths
• Provides birth and death certificates for the current year and one previous year (Older certificates are available from the Santa Clara County Clerk-Recorder or call 408-299-2481)
• Issues permits and transit letters for disposition of human remains
• Provides forms and assistance to persons who wish to amend records
• Provides forms and acts as a qualifying witness for establishing paternity
